Sending a refill request for refillable medications has never been easier than it is now, using MyOnlineChart.com. Now you don’t even have to pick up the phone, or visit your doctors office in order to make your next appointment. You may also view the details of your past appointments and see when upcoming appointments are scheduled.
If you have an urgent medical matter, you should not use the services of MyChart to send messages. Urgent matters should be directed to your doctors office or if needed, 911 should be called.
New User Sign up
- To start creating a new myonlinechart account, click the button titled “Sign up Now” located towards the right-hand side of the homepage.
- Enter your activation code as found on your enrollment letter. After the sign up process has been completed, you will no longer be required to use this code.
- Next, enter the last four digits of your Social Security number.
- Enter your birthdate in the format shown using four digits for the year.
The three steps mentioned above, are required in order to create a valid account. Once this information has been entered correctly and verified, click “Next” to finish the registration process.
Mobile apps are now available through the Google play store as well as the Apple app store to allow members to access their My Chart member account using mobile devices went on the go. You’ll find the links to download these mobile apps to your phone or mobile device located in the lower right-hand corner of the homepage.
